I can only answer for the parts of Tuscany that we stayed in - we had no problem with just english.  We stayed in Chianti but did many day trips to the nearby areas.  We went to Siena, Pisa, Florence, and a few other areas near by there.  We were nervous about not speaking or understanding any italian, but we had no issues at all.
